IF YOU ARE SICK:
If you are showing or feeling sick from ANY illness, do not enter Fine Cuts. Please call to cancel your appointment or do so online.

Confirm And Agree:
By creating an appointment at Fine Cuts, whether Online, by telephone or in person, you confirm and agree to the following liability waiver and health statements.

On my behalf, I hereby release, covenant not to sue, discharge, and hold harmless Fourth and Union Hair, Inc. DBA Fine Cuts, its employees, agents, and representatives, of and from the Claims, including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating thereto. I understand and agree that this release includes any Claims based on the actions, omissions, or negligence of Fine Cuts, its employees, agents, and representatives, whether a COVID-19 infection occurs before, during, or after visiting Fine Cuts.

Health Statements

You affirm that you have not experienced any of the below symptoms in the last 14 days.

  • • Fever
  • • Body aches and pains
  • • Coughing
  • • Sore throat
  • • Unexplained sores on bottom of feet
  • • Shortness of breath
  • • Runny nose (non-allergy related)
  • • Chills with or without aches
  • • Unusual fatigue

• That no one in your household experienced any of the above symptoms in the last 14 days.

• That you have not been in contact with anyone that has been confirmed as having Covid 19 in the last 14 days.

• That you have not returned from abroad in the last 14 days?

You understand the importance of notifying Fine Cuts using the below contact information if you experience any of the above symptoms up to 14 days after your service.

That if you or a member of your household experience any of the symptoms related to Covid 19 or are diagnosed with Covid 19 within 14 days of you having a service at Fine Cuts.
